Visitor/Clubhouse Registration Form

Under the current covid-19 government guidleines, we have a responsibility to collect contact details for each visitor to the club. This will then be kept for 21 days in case they are required for the NHS Test and Trace procedures. 

We require each vistor to complete the form by clicking on the link below which should take less than one minute to complete.
